Employee User Account Request

Description:

 

  • PLEASE SUBMIT ACCOUNT REQUESTS AT LEAST TWO WEEKS (10 BUSINESS DAYS) BEFORE THE NEW USER'S FIRST WORKING DAY
  • User account requests are to be submitted by Supervisors or Designee only. Requests not submitted by a Supervisor or Designee will not be processed.
  • After the account has been created, the user's new account credentials and information will be available by making an appointment with IT. Credentials are available to be picked up at the IT Help Desk in building 1200, or over the phone.
  • A separate ticket must be submitted for each/every individual user for which you are requesting a new User Account.
