Employee User Account Deactivation Request

  • For supervisors (or designees) of Short Term Employees, Student Assistants, Contractors, and Volunteers ONLY
  • Accounts assigned to these employees will normally be deactivated at the end of the day on their provided end date.
  • If an employee ends their employment before their expected end date, use this form to notify IT that their account should be deactivated.

 

Please note: This form is not for requesting a change/extension to an employee's end date. To request an extension, create a User Account Extension request.
