Staff/Faculty User Account Request

Description:

  • PLEASE ALLOW UP TO TWO WEEKS FOR THE COMPLETION OF REQUESTS.
  • Requests are to be filled out by Supervisors or Designee only. Requests not filled out by a Supervisor or Designee will not be processed. 
  • Information Technology offers network access and email accounts to all full and part-time staff and faculty. Requests are processed weekly.  After processing completes, the user's new account information will be available for pickup at the IT Help Desk, in building 1200.
  • Submit a separate form for each/every individual user that you are requesting a new Network Account.

Please note: this form is not for requesting access to Colleague.  For Colleague access, please fill out a Colleague Account Request form.
