Software Request

Prerequisites:

  • Obtain approval from your supervisor or department administrator.

  • Request a Higher Education Community Vendor Assessment Toolkit (HECVAT) form from the vendor for software or cloud services that handle sensitive data (e.g., legal statistics, address information, financial information, medical records, biometric data).

  • Additional information may be requested before the final review.

Purchasing Process:

  • Once approved, contact the purchasing department if procurement is required.

General Guidelines:

  • Use this form to request approval and installation of software and cloud services

  • Pre-approved software (Adobe Creative Cloud, Microsoft Visio, etc) can be installed on Cabrillo-owned computers for any eligible employee.

  • New software purchases, and purchases or renewals of cloud-based services, are subject to Information Security review before they can be installed, renewed, and used. You will need to gather additional information from the software vendor and provide it to IT. Requests must be approved by the Information Security Officer.
